8.03 Life Insurance and Accidental Death and Dismemberment

Purpose:

The College provides specified levels of group Life Insurance and Accidental Death and Dismemberment insurance (AD&D) to benefit-eligible employees, at no cost to the employee. This policy describes those benefits.

Procedure:

Eligibility/Enrollment

Enrollment is available to all continuing full-time (either full-year or academic year) and half-time (full-year only) employees.

Upon hire into a benefit-eligible position, eligibility starts on the first day of the month coinciding with or following employment.

Group life insurance protection is provided for employees in an amount equal to base annual salary rounded to the next higher $1,000. Coverage does not exceed $50,000 (the maximum non-taxable amount of group coverage allowed under current federal law).

On June 30th following an active employee’s 70th birthday, the amount of life insurance is changed to 65% of salary, up to a maximum of $32,500.

The group AD&D benefit is determined according to a loss schedule formula based upon the amount of the life insurance benefit.

Costs

The premiums for group life insurance and AD&D are fully paid by the College.

Termination of Coverage

Life insurance coverage ceases on the last day of employment, with the following exception. For employees who retire at age 60 or later, who were employed on a full-time basis prior to December 1, 1977, the College continues the life-insurance benefit at a reduced amount equal to 25% of the insurance in effect at retirement, up to maximum of $3,750.

Employees who leave Trinity may convert their group life insurance benefit into a non-group plan within 30 days of the expiration of coverage. Evidence of insurability is not required. Forms for this conversion are issued from the Human Resources Department.

Accidental Death and Dismemberment coverage ceases on the last day of employment.

Employees on Military Service Leave who have already qualified for life insurance and AD&D, continue coverage while on approved leave.
